Episode 82: Lost faith in surrogates yet? ACCORD challenges beliefs again

In episode 82, James and Mike discuss the latest results from the ACCORD trial. While the results really showed no difference between the groups, these results should make a difference in what you do. During much of the podcast, Mike does an unbelievable impersonation of a parrot but, we aren't sure if anyone will notice that difference.

The "treat and forget" statin

The "treat and forget" statin approach is formalized and validated in a very interesting simulation study published in Annals of Internal Medicine. Hayward et al compared the usual treat-to-target approach with a risk-driven stategy (moderate dose/potency statin for moderate CAD risk, high dose/potency statin for high CAD risk) with no subsequent agonizing over target levels. The estimated benefits of the risk-driven strategy were prevention of more CAD events while treating fewer persons with high-dose statins than
the treat-to-targetapproaches.
